Home Health

Olympic Medical Home Health is a Medicare-certified, state-licensed agency dedicated to providing comprehensive, high-quality healthcare in homes throughout Clallam County.

We provide short-term skilled nursing and therapy services to people who are homebound and recovering from an injury, illness or surgery. Wound care patients, stroke survivors, heart failure patients and people with Parkinson’s disease are just some of those we help in our community.

Our home health team

We follow all regulations for home health agencies, including licensing and background checks for our staff. Our team has earned praise from our patients for their courtesy, respect and listening skills.

Our team includes registered nurses who are skilled in chronic disease and palliative care (symptom relief). Depending on your needs, you may also receive home care from other providers, such as a physical, speech or occupational therapist; medical social worker; or a home health aide.

How we provide in-home health care

Our care starts with a careful assessment of your health needs. Then we will design a care plan that meets your doctor’s approval.

After that, a nurse or other provider will make planned visits to your home. Many of our home health patients receive visits once or twice a week for up to a few months. Depending on your needs, treatments and services we provide in your home may include:

  • Medication management, including assistance with any IV (intravenous) drugs.
  • Rehabilitative therapies, such as mobility and balance exercises.
  • Teach self-management of chronic disease
  • Wound care.

Along with your treatment, our team will also teach you and your caregivers what you need to know to manage your care on your own after our service ends. If you need additional support, we can also connect you to other resources in the community.

Services offered

Olympic Medical Home Health (OMHH) accepts referrals for Clallam County patients 12 years and older, with skilled medical or therapy needs that align with services that meet acceptance criteria. OMHH evaluates the anticipated needs of a referred prospective patient to ensure they meet participatory guidelines. Caseload, case mix according to the staffing levels, skills and competencies of OMHH staff are also considered during review. Qualifying patients are accepted when services can be safely provided regardless of race, religion, nationality, marital status, sex, ethnicity, sexual orientation, financial support, or disability, unless the disability cannot be managed effectively by OMHH.

Service duration

OMHH services are certified in 60-day increments. Discharge from service may occur prior to 60 days when services can no longer meet patient needs of skilled care, if goals of care are met, or in the event services cannot be provided in a safe manner. In the event skilled needs may extend beyond the 60-daycertification, the need for service is evaluated by the patient care team.

Service limitations

OMHH reserves the right to decline service referrals when skilled needs exceed the capacity of clinician ability to provide safe care, or if clinician safety risks are identified. OMHH performs a chart review and screening for appropriateness of OMHH referrals, to identify patient skilled needs, and to identify risk factors . This process begins in Intake, and in coordination with the RN Transitional Care Coordinator, PCC, Clinical Managers or Director as needed.

  • OMHH does not provide service/accept referrals for patients with Endotracheal Tubes, Nasogastric Tubes, or Tracheostomies
  • OMHH does not provide service/accept referrals for patients when it is determined through OMHH referral and chart review that their skilled needs require a higher level of care than can be provided by OMHH.
  • OMHH does not provide service/accept referrals when providing in home skilled assessments and therapies carry an increased risks to patient safety or clinician safety
  • Patients residing in facilities or institutions that provide on-site skilled care will not be accepted when ordered services are a duplication of services already being provided
  • Patients receiving and attending outpatient therapy will not be accepted onto HH services, to avoid duplication of services
  • Patients receiving services of Medicare Hospice will not be accepted onto HH services, to avoid duplication of services.
