We wish to acknowledge and honor the basic rights of individuals for their independence of expression, decision making, and personal dignity; as related to care and treatment.

The following rights and responsibilities will be observed for each patient; and when appropriate for the patient's legally authorized representative, parents, or guardian.

Right to:

  • Access to non-discriminatory care.
  • Respect and dignity.
  • Receive care in a safe, non-abusive setting.
  • Information on diagnosis, treatment prognosis.
  • Participation in care decisions to consent, refuse, or transfer.
  • Authorize and expect compliance with Advance Directives.
  • Have a family member or representative & personal physician notified of admission.
  • Assessment, intervention, and education for pain.
  • Freedom from restraints, seclusion unless medically necessary.
  • Personal privacy and confidentiality of clinical records.
  • Information on hospital regulations and policies.
  • Information on the professional identity & business relationship of care providers.
  • Explanation of charges.
  • Timely response to grievances.

Responsibility to:

  • Provide information on medical history, symptoms, and medications.
  • Request clarification of health status and treatment.
  • Follow treatment plans, or accept consequences of refusing.
  • Respect the rights and property of others.
  • Provide copy of Advance Directives, when possible.
  • Follow hospital regulations and policies provided.
  • Assure financial obligations.
  • Express concerns & grievances allowing for timely resolution.
