rights and responsibilities will be observed for each patient;
and when appropriate for the patient's legally authorized representative,
parents, or guardian.
Right to:
- Access to non-discriminatory care.
- Respect and dignity.
- Receive care in a safe, non-abusive
setting.
- Information on diagnosis, treatment
prognosis.
- Participation in care decisions
to consent, refuse, or transfer.
- Authorize and expect compliance
with Advance Directives.
- Have a family member or
representative & personal
physician notified of admission.
- Assessment, intervention, and education
for pain.
- Freedom from restraints, seclusion
unless medically necessary.
- Personal privacy and confidentiality
of clinical records.
- Information on hospital regulations
and policies.
- Information on the professional
identity & business
relationship of care providers.
- Explanation of charges.
- Timely response to grievances.
Responsibility to:
- Provide information on medical
history, symptoms, and medications.
- Request clarification of health
status and treatment.
- Follow treatment plans, or accept
consequences of refusing.
- Respect the rights and property
of others.
- Provide copy of Advance Directives,
when possible.
- Follow hospital regulations and
policies provided.
- Assure financial obligations.
- Express concerns & grievances
allowing for timely resolution.
