Assesses, plans, implements, coordinates and evaluates the nursing care of a group of 15 – 25 patients in their place of residence. Identifies and addresses patient care/teaching/learning needs and documents correctly. Involves family and significant others in planning and teaching processes. Associate’s degree required. B.S.N. preferred. Minimum of one year recent experience in home health, acute or long-term care. Current WA State RN license. Weekends, holidays, on call.
