Provides assistance to patients whose psychosocial needs are interfering with or delaying effective treatments provided by the Home Health interdisciplinary team. Master’s degree from an accredited school of social work. Minimum two years social work experience preferred with one year working with the elderly or in a health care setting. Registered as a counselor in the State of Washington and as Licensed Independent Clinical Social Worker.
